FormVault provides athletic form management services for schools and parents. By using FormVault, you agree to be bound by these Terms of Service.

1. Acceptance of Terms

By accessing or using FormVault, you agree to these Terms of Service and our Privacy Policy. If you do not agree, please do not use the service. These terms apply to all users, including school administrators, coaches, and parents.

2. Description of Service

FormVault is a software-as-a-service (SaaS) platform that enables schools to manage athletic participation forms, collect digital signatures, and communicate with parents and athletes. Features include form creation, digital signing, campaign messaging, and reporting.

3. Account Responsibilities

School administrators are responsible for maintaining the security of their accounts and for all activities that occur under their accounts. You must notify us immediately of any unauthorized use. FormVault is not liable for losses caused by unauthorized account access.

4. Data Privacy

We collect and process personal information as described in our Privacy Policy. By using FormVault, you consent to the collection and use of information in accordance with that policy. Schools are responsible for obtaining appropriate consents from parents and athletes for data they submit.

5. FERPA Compliance

FormVault is designed to support schools in meeting their obligations under the Family Educational Rights and Privacy Act (FERPA). Schools remain the data controllers for student records stored in FormVault. We act as a service provider processing data on behalf of schools and do not disclose student information to third parties except as required to provide the service or as permitted by law.

6. Acceptable Use

You agree to use FormVault only for lawful purposes and in accordance with these Terms. You must not use FormVault to collect or transmit any information that violates applicable law, infringe on intellectual property rights, or interfere with the service's operation.

7. Subscription and Billing

Access to FormVault requires a paid subscription. Subscription fees are billed in advance on a recurring basis. You may cancel at any time; however, refunds are not provided for partial billing periods unless required by law.

8. Limitation of Liability

To the maximum extent permitted by law, FormVault shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of the service. Our total liability shall not exceed the subscription fees paid in the three months preceding the claim.

9. Changes to Terms

We may update these Terms from time to time. We will notify you of significant changes by email or by posting a notice on the platform. Continued use of FormVault after changes constitutes your acceptance of the updated Terms.
